From 9 October 2017 we will be using a car parking enforcement service

From 9/10/17 we will be using a car parking enforcement service so that cars not registered on the touch screen in reception will be fined, we are doing this to free up parking spaces in our car park for our patients, rather than provide free parking for hospital staff or shoppers in Headington.

Room Changes 

Now the new building is open, we have renumbered the consultation rooms.  Please note the nursing rooms are numbered 1-4 and the GP rooms 5-16.  There are notices in the waiting room to help guide patients around the building. 

New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.

You will need to complete a registration form (GMS1) and a medical  questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. All forms are available on the above "Registering Online" tab.

Medical treatment is available from the date of registration. Please contact reception for further information.
