You can use this form to contact us with any  non-urgent queries. We will get back to you within 3 working days. Our default method for communication is email, if you would prefer to be contacted by telephone please indicate this on the form. 

If you have a Covid-19 related query please look at the information page on our website before contacting us with a query. If your employer is asking for evidence of fitness for work, there is a letter on the page that you can download and share with your employer. We cannot provide individual letters for employers regarding fitness for work. This should be discussed with your employer and your occupational health team.
